Bronchogenic cyst - mediastinal

Presentation

Mild chest pain.

Patient Data

Age: 40 years
Gender: Male
x-ray
  • large cystic middle and posterior mediastinal lesion

  • splaying of the carina

  • separate from vertebral borders and heart contour

  • clear lungs

  • confirms a middle and posterior mediastinal lesion

    • located immediately below the carina

    • splaying of the carina and mass effect on the right main bronchus

    • non-calcified

    • cystic with no solid components

    • imperceptible very thin wall

Case Discussion

Findings in this case are most consistent with a bronchogenic cyst. Esophageal duplication cyst and atrial pericardial cyst are also reasonable differential diagnoses. The immediate subcarinal location and imperceptible thin wall are features in support of bronchogenic cyst.
