Which testis is abnormal?
The right testis is abnormal. When compared to the left it is diffusely hypoechoic and hypervascular, both signs of orchitis.
What is the structure adjacent to the right testis.
A septated hydrocoele. These are often seen in conjunction with orchitis.
The right testis measures 5 x 3 x 3 cm. the testis is diffusely hypoechoic with heterogeneous echotexture and increased internal vascularity consistent with history of orchitis.
The right epididymal head demonstrates increased vascularity when compared to the left.
A large septated right sided scrotal collection is seen measuring 7 x 4 x 5 cm in diameter. It is predominantly anechoic with internal septations.
The left testis measures 5 x 4 x 3 cm. It demonstrates normal echotexture and echogenicity, with no focal lesions seen. The left epididymis is normal, with a small epididymal cyst. A small left sided hydrocele is noted.
Conclusion:
- Right sided epididymo-orchitis, with a large septated hydrocele. No evidence of abscess formation or testicular infarction.
- Small left sided hydrocele. The left testis is normal.
