Why is the position of the penis critical when performing a retrograde urethrogram / ascending urethrogram for possible urethral stricture?

The penis is held taut at the 9-10 o'clock position so that the entire length of the urethra can be seen without foreshortening, which would artificially decrease the size of the stricture. The patient is also turned to the RPO position to minimize the amount of overlap between pelvic bones and the urethra.

Presentation

Patient with known urethral stricture post radical prostatectomy for prostate adenocarcinoma and radiation therapy to the pelvis for recurrence. Gradually decreasing urine stream. Hesitancy, dysuria, and nocturia. Re-evaluate stricture.
