The patient was likely immunosuppressed due to congenital rubella infection. One month before this CXR, sputum culture confirmed Pneumocystis jiroveci infection. After this CXR, lung biopsy showed interstitial fibrosis and inflammation which can be seen both in rubella and Pneumocystis jiroveci infection.
