What LI-RADS category would you assign this mass?
First before assigning a category... is this patient at risk for HCC? Yes: because of hep C. Then look at its characteristics: "new" 13 cm mass, hypervascular enhancement with washout, pseudocapsule... check, check, check, check. This is a LI-RADS 5B lesion: definitely HCC (100%).
What should you look for next?
A second lesion in the contralateral lobe. Portal vein thrombus. Metastases.
Dynamic postcontrast sequence after 10 ml of Magnevist.
