Large OCD defect with separated osteochondral fragment i.e. cortical bone with low signal intensity and overlying attached articular cartilage. Note clear fluid signal sitting between the fragment and underlying bone indicating separation. This situation therefore requires attempted repair (reattachment) or excision.
