20 yr old male patient, presented with attacks of mild tachypnia, no other symptoms what are the findings on his chest X-ray?
A curvilinear opacity seen passing from the right hilum inferiroly to the level of the diaphragm, it is increasing in width inferiorly, the right lung appear small in size compared to the left lung (right ribs crowding and elevated right hemidiaphragm)
What are the findings on his chest CT?
The right inferior pulmonary vein is seen draining into the inferior vena cava, the right lung is small in size with absent right middle lung lobe resulting in abnormal cardiac orientation
What is your diagnosis ?
Scimitar syndrome (hypogenetic lung syndrome or pulmonary venolobar syndrome)
What are the common sites of draining of the Scimitar vein ?
1) the inferior vena cava 2) the suprahepatic veins 3) the right atrium
An anomalous pulmonary vein draining into the inferior vena cava consistent with partial anomalous pulmonary venous return (Scimitar syndrome), is noted. The right lung is smaller than the left lung resulting in an abnormal cardiac orientation, the right middle lung lobe is missing (congenitally absent).
