This case shows the evolution of a malignant ICA (i.e. MCA-ACA) infarction to the point where cerebral edema is so great that there are at least four types of herniation involved:

• subfalcine
• inferior transalar (trans-sphenoidal)
• descending transtentorial
• tonsillar
