Huge cardiomegaly due to massively enlarged left atrium with dilated pulmonary veins, and compressive atelectasis of the adjacent lung bilaterally. Other cardiac chambers are compressed and displaced.

IVC and hepatic veins are severely dilated. There is marked periportal oedema (periportal halo).
