What are the differentials for nonmalignant diffuse bony sclerosis and their associated radiographic signs?
1. Hyperparathyroidism which is associated with the rugger-jersey sign. 2. Osteopetrosis which is associated with the sandwich vertebral body. 3. Paget disease of the bone which is associated with picture frame vertebral body.
Why might a patient with osteopetrosis suffer from pancytopenia and splenomegaly?
Sclerosis of the bone medulla can lead to pancytopenia due to impaired haematopoiesis. Extramedullary haematopoiesis can occur in response to this and a common location is the spleen which can, in turn, lead to splenomegaly.
Considerable cardiomegaly.
Lungs and pleural spaces clear with no features of APO.
Extensive sclerosis affecting all visible bones.
