Congenital lobar overinflation (previously called congenital lobar emphysema) usually affects the upper lobes, but can affect any lobe. CXR can underestimate the degree of overinflation.

The importance of always assessing the bones of young children (especially ribs) is evidence in this case. Posterior rib fractures are highly suggestive of inflicted non-accidental injury.
