CT
There is an expansile destructive lytic mass centered within the central skull base and posterior sphenoid paranasal sinus. The mass demonstrates linear spicules of calcification/ossification. It extends cranially to result in demineralization of the planum sphenoidale. It also results in demineralization and effacement of the sella turcica. There is caudal extension into the posterior nasopharynx and expansile changes and infiltration of the left lateral pterygoid musculature. There is ventral extension to efface portions of the posterior ethmoid paranasal sinus air cells.
Effacement of the cavernous sinuses (left more so than the right), left superior orbital fissure, left inferior orbital fissure, and left pterygopalatine fossa is seen. There is also destruction of the left pterygoid plate. The left foremen ovale and foreman rotundum are also effaced. Both the left infraorbital foramen and right pterygopalatine fossa appears to be within normal limits. The intraconal and extraconal fat of both orbits is preserved.
The central skull base mass extends into the cavernous sinuses bilaterally (left more so than the right) with subsequent displacement of the left ICA superiorly.
