Ovarian torsion

Discussion:
  • Findings: The left adnexal cystic structure is torsed in abnormal location (sits abnormally high in the midline/ right lower quadrant - enlarged ovary in an abnormal location).

  • Objectives:

    • To review female pelvic anatomy

    • To learn the signs that should prompt suggestion of ovarian torsion on CT scan

  • Protocolling:

    • Ultrasound test of choice when torsion suspected clinically

    • Usually seen when CT performed for non-specific abdominal pain

  • Key Points:

    • Anatomy:

      • Review female pelvic anatomy on CT

      • Follow ovarian vessels to help identify region of ovaries

    • CT Findings:

      • “Twisted pedicle” is specific but only occasionally seen

      • ENLARGED ovary (often with a mass) in ABNORMAL location

      • Edematous ovary with peripheral follicles (difficult to resolve on CT)
