Intra-ventricular masses

Adults:

  • Ependymoma
    • Posterior fossa more common in children; supratentorial more common in adults
    • Associated with NF2
    • CT: coarse calcification, cystic areas, heterogenous enhancement
    • MRI: T1 iso- to hypo-, T2 hyper-, SWI blooming, diffusion restriction, heterogenous enhancement
  • Sub-ependymoma
    • Older patients
    • CT: similar to ependymoma except usually no enhancement
    • MRI: similar to ependymoma except usually no enhancement or diffusion restriction.
  • Meningioma
  • Metastases
    • RCC, melanoma, breast, lung, papillary thyroid primaries.
  • Colloid cyst

Children:

  • Ependymoma (see above)
  • Choroid plexus papilloma
    • Lateral ventricles children; 4th ventricle adults
    • CT: lobulated and frond-like appearance, hyperdense, homogenous enhancement
    • MRI: T1 iso, T2 iso- to hyper-, vivid homogenous enhancement
  • Choroid plexus carcinoma
    • Similar to above except may show parenchymal invasion and heterogenous contrast-enhancement.
  • Teratoma
  • Atypical Rhabdoid tumour

Other:

  • Sub-ependymal giant cell astrocytoma (SEGA)
    • Associated with tuberous sclerosis
    • Mass near foramen of Munro
    • Calcification common, marked contrast-enhancement.
