Acute sacroiliitis - unilateral

Abnormal subchondral bone marrow signal involves the left sacroiliac joint and displays the low signal on T1 and high signal on T2WI and fat-suppressed images with post-contrast enhancement due to the acute inflammatory process (osteitis).
Preserved joint space of ligaments and synovial parts of the right sacroiliac joint.
Normal shape and signal intensity of the muscle attachments. No enthesitis.
Case Discussion
MRI findings are consistent with left-sided acute sacroiliitis.
