Anterior cruciate ligament tear and Bucket handle tear of medial meniscus

Diagnosis almost certain

Presentation

Twisting injury of the knee.

Patient Data

Age: 50 years
Gender: Male

Complete tear of anterior cruciate ligament.

Bucket handle tear of medial meniscus, with medial transposition of torn fragment giving double posterior cruciate ligament appearance (double PCL sign) and fragment in notch sign. Displaced meniscal fragment anteriorly giving double delta sign.

Moderate knee joint effusion. 

Moderate osteoarthritic changes are seen as areas of chondral loss, and subchondral cystic changes.

Intrasubstance high T2 signal along the medial collateral ligament reflecting grade 1 injury.

Diffuse subcutaneous edema.
