3 days of lower abdominal pain. No bowel symptoms. Globally tender.
Patient Data
A large ovoid focus of low attenuation surrounded by omental fat stranding was found on CT. The anterior margin sigmoid colon of the patient indicates acute epiploic appendagitis. This focal point was further enhanced using a Z-Effective map on spectral CT with contrast to show a lack of enhancement of the epiploic appendagitis against enhanced fat stranding.

Case Discussion
The patient presented with lower abdominal pain, which is in line with usual symptoms of epiploic appendagitis. Patients generally present with acute, sharp pain in the abdomen without symptoms such as fever, nausea or diarrhea and can be mistakenly diagnosed as diverticulitis 1.
