Fifth finger proximal phalanx fracture with A2 pulley injury

Presentation

Fifth finger trauma two months ago and now complained of fifth finger flexion deformity.

Patient Data

Age: 45 years

Fracture lines at the base of the fifth finger proximal phalanx base with intraarticular extension

A2 pulley injury associated with increased distance between the flexor tendon of the fifth finger and the proximal phalanx

There is also edema deep to the flexor tendon at the proximal phalanx, with “bowstringing” of the tendon

Bone marrow edema /bruise along with fifth digit distal phalanx

Fifth digit subcutaneous edematous changes and swelling.

Case Discussion

The recent patient had an old trauma two months ago and complained of fifth finger flexion deformity. Regarding the chronicity of trauma and MRI findings, A2 pulley injury in the background of the fifth finger proximal phalanx fracture is the main differential diagnosis.
