Fat (T1WI and T2WI hyperintense) signal intensity is seen in filum terminale at the L4-L5 level.
Conus medullaris is low-lying (lower end of L3 vertebra).

Imaging features suggest filum terminale lipoma with a tethered cord.
Filum terminale lipoma is a relatively common and incidental finding that is asymptomatic in most cases. However, it can be associated with spinal dysraphism or tethered cord.
