Flexor carpi radialis tunnel syndrome

Diagnosis almost certain

Presentation

Persistent volar radial wrist pain over 6 months.

Patient Data

Age: 40 years
Gender: Male
  • mild radio scaphoid joint effusion

  • fluid signal within the flexor carpi radialis tendon (FCR) sheath due to tenosynovitis

  • the distal FCR tendon within the hand is flattened and the adjacent trapezial ridge exhibits mild reactive marrow edema

  • bone marrow edema along with hamate hook and pisiform bones

  • the fluid signal within the flexor pollicis longus tendon sheath due to tenosynovitis

  • abnormal intrasubstance increased fluid signal along with triangular fibrocartilage proximal and distal lamina related to TFCC sprain

  • mild distal radioulnar joint effusion

Case Discussion

MRI features are consistent with flexor carpi radialis tendinopathy and trapezium bone marrow edema. Flexor carpi radialis (FCR) tendinopathy presents with pain in the volar radial wrist. It usually develops due to chronic overuse, particularly in areas of the tendon that are exposed to rough surfaces.
