Presentation
Pelvic pain. Had a renal graft for end-stage renal failure 7 years ago.
Patient Data
Complete duplication of uterine horns as well as the cervix with no communication between them in keeping with uterus didelphys.
Focal thickening of the endometrium-myometrium junctional zone involving both uterine horns of anterior location on the right and posterior location on the left, seen as an ill-defined area of intermediate signal on T1, low signal on T2 with foci of high signal (menstrual hemorrhage into the ectopic endometrial tissues), compressing the endometrial cavity most consistent with focal adenomyosis.
Numerous intramural leiomyomas (Figo 4 and 5) are noted.
The transplanted kidney is seen in the left iliac fossa.
Case Discussion
MRI features of focal adenomyosis with numerous intramural leiomyomas in uterus didelphys.
