Presentation

Chronic shoulder pain with limited movements.

Patient Data

Age: 40 years
Gender: Male
  • full-thickness tear of the supraspinatus tendon critical zone fibers associated with the retraction and abnormal orientation of the superior portion of torn fibers, making a two-layered structure of the supraspinatus tendon on coronal and sagittal images

  • abnormal intrasubstance increased fluid signal and thickening along with supraspinatus tendon posterior fibers, infraspinatus, and subscapularis tendons, as well as the long head of the biceps tendon related to tendinosis

  • mild effusion at the glenohumeral joint and subacromial/subdeltoid bursa

  • mild capsular hypertrophy and degenerative changes along with AC joint

  • type II acromion

Case Discussion

The findings are compatible with a Fosbury flop tear of the supraspinatus tendon, and the accumulation of fluid in the subacromial bursa supports the diagnosis.
