Diagnosis almost certain

Presentation

Bilateral hip pain for 2 years.

Patient Data

Age: 14 years
Gender: Female

Sclerotic changes of both femoral epiphysis with the crescent sign.

Right hip

Sclerotic changes of the right femoral epiphysis with a crescent sign involving more than 50% of the femoral head

Loss of the normal bright signal in both T1WI and T2WI in the involved regions.

No significant cystic changes are noted in the metaphysis.

Left hip

Sclerotic changes of the left femoral epiphysis with a crescent sign involving less than 50% of the femoral head.

Loss of the normal bright signal in both T1WI and T2WI in the involved regions.

Case Discussion

The features suggest bilateral Legg-Calvé-Perthes disease: Right group B & left group A according to Salter-Thompson classification.
