Presentation
obstructive jaundice since few days, referred for further assessment of diffuse intrahepatic biliary duct dilatation seen on ultrasound.
Patient Data
Two gallstones are seen near the gallbladder neck, the largest of which is exerting extrinsic compression on the adjacent common hepatic duct, causing moderate diffuse intra-hepatic bile ducts dilatation.
Incidental note is made of fundal adenomyomatosis and splenomegaly.
Annotated image: the red arrow indicates the extrinsic compression of the common hepatic duct exerted by a large gallstone impacted near the gallbladder neck.
Case Discussion
MRI findings are consistent with Mirizzi syndrome.
