Ovarian dermoid cyst

Presentation

Abdominal pain and irregular menses.

Patient Data

Age: 45 years
Gender: Female

There is a 10 x 12 x 16 cm large multiloculated cystic lesion along with the right adnexa occupying the pelvic cavity and making mass effect upon the adjacent structures.

The lesion exhibits a mural nodule (4 x 5 x 6 cm) of inhomogeneous high signal on both T1WI/T2WI with attenuation of the fatty component on fat-saturated sequences (Rokitansky nodule).

On post-contrast sequences, the lesion showed a thin smooth wall and multiple internal septa enhancements without enhancing invasive soft-tissue components.

Case Discussion

There is a high probability of a large mature cystic teratoma based on the MRI imaging findings and it is classified as MRI ORADS III.
