Pigmented villonodular synovitis of knee

Case contributed by Aneesh Km

Presentation

Knee pain.

Patient Data

Age: 30 years
Gender: Male

Diffuse synovial thickening was noted involving the knee joint. Thickened synovium is hypointense on T2 and shows multiple irregular low T2 foci. Multiple T2 hypointense signal loose bodies are also noted in suprapatellar bursa.

Both medial and lateral tibial and femoral condyles show large cysts with heterogeneous signals and low T2 signal intensity rim. The hypointense T2 signal foci described are possibly due to hemosiderin contents.

Case Discussion

This case illustrates the typical appearances of pigmented villonodular synovitis (PVNS) of the knee.

Editor's note: Per the 2020 WHO Soft Tissue and Bone Tumors Classification (5th ed.), the recommended terminology is tenosynovial giant cell tumor with pigmented villonodular synovitis no longer recommended (although remains in common use).
