Presentation

History of AIDS.

Patient Data

Age: 45 years
Gender: Male

Multiple multifocal asymmetrical non-enhancing high T2/FLAIR white matter signal intensities are noted in the left periventricular, left centrum semiovale, juxtacortical white matter (involving U-subcortical fibers), medial aspects of the right occipital lobe, and right basal ganglia without restricted diffusion or mass effect.

Multiple high T2 punctate foci in the left periventicular and semiovale, giving the Milky Way sign.

Case Discussion

The patient's imaging findings and history of AIDS suggest progressive multifocal leukoencephalopathy.

The patient underwent a PCR test and he was positive for the JC virus, which confirmed the diagnosis.

Milky Way sign is associated with a variety of disorders but are most commonly seen with PML.
