The patient fell down stairs one week ago, with tenderness at the head of the fourth metatarsal. Possible fracture. Patient has known psoriatic arthritis.

Midfoot alignment is maintained.
Ankylosis of the second to fifth proximal interphalangeal joints.
Pencil-in-cup deformities of the second to fourth proximal interphalangeal joints.
Imaging features in keeping with known psoriatic arthritis.
