Rectus femoris heterotopic ossification

Presentation

Progressive right hip pain and restricted range of motion with a grinding sensation noted during deep squats with a remote history of injuries that were described as “avulsion type” and located at the anterior inferior iliac spine (AIIS).

Age: 70 years
Gender: Male

Calcification of elongated morphology near the superior lip of the acetabulum from the anterior inferior iliac spine (AIIS). There is also subtle lamellar calcification of the soft tissues in the posterior compartment of the thigh, sequelae of myositis ossificans of previous muscle injury. The coxofemoral joint is normal.

Case Discussion

Athletes are particularly susceptible to chronic microinjury and acute tendon avulsion, resulting in rectus femoris heterotopic ossification, as in this case. Heterotopic ossification (HO) of the anterior inferior iliac spine is typically the result of avulsion injury of the rectus femoris muscle. The HO can lead to a decrease in the hip range of motion and function, and as in this case, extra- articular subspine impingement of the hip.
