Slow-growing scrotal mass
Patient Data
Right inguinal-scrotal fat containing (T1 and T2 hyperintensity; loss of signal on fat suppression sequences) mass encompassing the spermatic cord, with interspersed enhancing septa of various thicknesses.
Anatomopathological report (translated from Italian):
Specimen: right orchifunicolectomy
Diagnosis: mesenchymal neoplasm with adipocyte differentiation [...] consistent with well-differentiated liposarcoma (lipoma-like; grade 1) of the spermatic funiculus according to WHO 2020 classification

Case Discussion
The patient underwent a right orchifunicolectomy confirming the diagnosis of spermatic cord liposarcoma.
Masses containing fat from the groin and scrotal regions are easily mistaken for inguinal hernias.
The contrast-impregnated soft-tissue components may point towards a neoplastic adipocytic lesion, whereas anteromedial cord displacement is more typical of hernias 1.
