Diagnosis almost certain

Presentation

History of esophageal carcinoma underwent surgical resection. Severe lower dorsal pain with weight loss, and fever two months after surgery.

Patient Data

Age: 55 years
Gender: Male

Abnormal bone marrow signal at the D10/11 level vertebral body displaying T1 hypo- and T2 hyperintense signals associated with irregular opposing endplates, destructed and irregular intervening disc with central T2 hyperintense focus, and lacking a central nuclear cleft.

Dehydration at D7/8 and D8/9 levels. 

Gastric pull-up surgery and moderate right-sided pleural effusion.
