Patient presents with long standing cough and mild respiratory distress. Patient is a smoker. No history of bronchiectasis.

Reduced volume of right lower lobe with bronchiectasis and reduced vascularity predominantly in basal segments. Subtle reduced vascularity and focal bronchiectasis also noted in lateral segment of right middle lobe. Compensatory hyperinflation of right upper lobe is seen. Left lung appears normal.
Case Discussion
Reduced volume of a lobe of a lung with diminished internal vascularity causing hyperlucency are features of Swyer James syndrome. Bronchiectasis may be seen in the affected lung segment.
