Tear of anterior cruciate ligament graft with ramp lesion

Presentation

Twisting injury; post ACL reconstruction

Patient Data

Age: 20 years
Gender: Male

Patient is follow up case of ACL reconstruction with evidence of femoral and tibial
tunnels in situ and surrounding marrow edema.

Near complete tear of mid segment/tibial attachment of anterior cruciate ligament graft (likely is seen. Few posterior fibers are possibly intact.

Partial tear of femoral attachment of medial patello-femoral ligament is seen.

Doubtful bony avulsion of deep fibers of medial collateral ligament is seen at medial femoral condyle with small bone fragment measuring approx 4.7mm adjacent to medial femoral condyle.
Lateral collateral ligament appears intact.

Vertical longitudinal tear of peripheral aspects of posterior horn of medial meniscus is seen adjacent to meniscocapsular junction, reaching superior as well as inferior articular surfaces.

Tear of antero-inferior popliteomeniscal fascicle is seen. Otherwise, lateral
meniscus is unremarkable. 

Thinning and loss of articular cartilage is seen along posterior aspect of medial
femoral condyle.

Focal delamination of articular cartilage is seen along lateral tibial condyle measuring approx 3mm in transverse and 3.5mm in antero-posterior dimensions.

Dorsal defect of patella is seen (developmental variant).
