Testicular adrenal rests

Presentation

The patient presented with a bilateral testicular mass.

Patient Data

Age: 20 years
Gender: Male

Both testes show bilateral, almost symmetrical hypoechoic lesions with minimal vascularity, eccentrically located, occupying the mediastinum.

Examination of the suprarenal fossae reveals a homogeneous isoechoic mass in the right fossa, and a mass in the left fossa.

Case Discussion

The patient is a known case of congenital adrenal hyperplasia. A testicular ultrasound was requested to rule out testicular adrenal rests.

Adrenal rests tumor typically locate near or within the rete testis. They originate from ectopic adrenal cells that migrate with the testicle from the urogenital ridge. After being stimulated by adrenocorticotropic hormone (ACTH), they enlarge and form masses.
