Presentation

Left hand amputation following generator fan blade cut.

Patient Data

Age: 45 years
Gender: Male

X-ray wrist left 2 views

Acute, severely comminuted traumatic fractures and amputations, as follows:

  • mid second, third, fourth and fifth metacarpals; the metacarpal bases are spared

  • third and fourth mid-middle phalangeal shafts

  • distal second proximal phalanx

Traumatic amputation at the first interphalangeal joint.

Acute posteriorly displaced obliquely oriented fracture of the ulnar styloid process.

Age indeterminate, mildly displaced triquetral fracture.

There is soft tissue irregularity and swelling associated with the traumatic amputations.

Case Discussion

Lacerations, crushes, and amputations represent a small spectrum of hand injuries. Due to the complexity of these injuries, there is no definitive treatment. Management of these complicated injuries includes pain control, preservation of sensibility, length, and healing optimization. In our case, stabilization and pain control were instilled immediately as a part of the treatment plan.
