Anteromedial meniscofemoral ligament

Patient Data

Age: 60 years
Gender: Female

Immediately anterior to the anterior cruciate ligament, a hypointense band is evident, which runs from the medial aspect of the anterior horn of the medial meniscus, following a similar path with respect to the anterior cruciate ligament, to the posteromedial aspect of the lateral femoral condyle.

Subluxation of the patella with respect to the trochlear groove. Focal soft tissue edema involving the superolateral aspect of Hoffa's fat pad just below the patella. These findings are suggestive of a patellar tendon-lateral femoral condyle friction syndrome.

Thinning of the patella cartilage with a small focus of subchondral edema, representing chondromalacia patellae.

Horizontal meniscal tear of the posterior horn of the medial meniscus, reaching the tibial surface.

Case Discussion

The anteromedial meniscofemoral ligament of the medial meniscus is a rare anatomical variant. The differential diagnosis is with an infrapatellar synovial plica (ligamentum mucosum). The way to differentiate it is because the anteromedial meniscofemoral ligament does not enter the infrapatellar fat pad or attach to the patella.
