Presentation

Intermittent painless post-menopausal vaginal bleeding.

Patient Data

Age: 55 years
Gender: Female
mri

A huge T2 intermediate signal intensity bulky barel-shaped cervical mass lesion shows the following:

  • size: 6 x 6.5 x 7 cm 

  • parametrial infiltration with disruptions of the normal low T2 cervical rim

  • the lesion is filling and markedly expanding the upper two-thirds of the vagina; intact lower third vagina

  • multiple enlarged bilateral internal iliac chains regional lymphadenopathy suggestive of metastatic nodes; the largest reaching 27 mm in the short axis on the left side next to common iliac biforcation

  • no extension into the pelvic sidewalls

  • no infiltration of the mesorectal fascia 

  • spared urinary bladder 

  • normal marrow signal pattern of the pelvic bones

Case Discussion

According to the FIGO staging system, the biopsy report for a cervical mass shows squamous cell carcinoma of the cervix, which can be classified as least Stage IIIC cervical cancer.
