Diastematomyelia, tethered cord and butterfly vertebrae

Presentation

Low back pain radiating to the left lower limb.

Patient Data

Age: 40 years
Gender: Female

Evidence of vertebral formation anomaly evidenced by multi-level butterfly vertebrae involving L2, L3 and L4 with incomplete fusion of spinous processes of L2 down to S1.
Evidence of diastematomyelia with thin hypointense, septum may be osseous or fibro-osseous cleft/spur at L2-3 level. Syringohydromyelia seen opposite to D11 level.
Scoliotic deformity of the lumbar spine is seen being convex to the left.
Mild forward slippage of L5 vertebral body over that of S1 secondary to bilateral L5-S1 facetal hypertrophic osteoarthritis.
Spondylodegenerative changes of the lumbar spine.

Evidence of tethered cord is seen ending at mid L4 level.

Case Discussion

Search for associated other CNS abnormalities, scoliosis (80%), Tethered cord (75%), vertebral body and urogenital anomalies.
