Discoid lateral meniscus

Presentation

Pain with intermittent locking of the right knee.

Patient Data

Age: 40 years
Gender: Female

On sagittal sequences, the lateral meniscus is visible in more than three images with an abnormal body width > 20 mm on the coronal sequence. An abnormal horizontal intrinsic high signal on PD fat saturation is seen within the meniscus (degenerative).

The medial meniscus appears normal. Mild joint effusion is seen.

MRI features of a discoid lateral meniscus with an intrameniscal mucoid degeneration

On coronal imaging, meniscal body width of 15 mm or more is typically considered diagnostic of discoid morphology. 

On sagittal imaging, a discoid meniscus is usually present if the meniscal body is seen on three or more standard sagittal slices

Discoid menisci have decreased collagen fibers and loss of normal collagen orientation, which predisposes to intrameniscal mucoid degeneration as in this case.
