Fetal tricuspid insufficiency with marked tricuspid regurgitation

Case contributed by Tushar Verma
Diagnosis almost certain

Presentation

The patient, gravida 3, came for a regular antenatal care (ANC) checkup.

Patient Data

Age: 30 years
Gender: Female

The examination reveals apparent cardiomegaly with minimal pericardial effusion noted. Marked regurgitation is observed along the tricuspid valve during systole, as seen on the cine video. Additionally, there is mild dilation of the right atrium secondary to tricuspid regurgitation

Case Discussion

Ultrasound examination reveals tricuspid regurgitation in the fetus, indicating the backflow of blood through the tricuspid valve during systole. This is visualised as a retrograde flow of blood from the right ventricle to the right atrium. Tricuspid regurgitation is commonly assessed using colour Doppler imaging, where the regurgitant flow appears as a jet of colour extending into the right atrium. It may indicate potential cardiac abnormalities and warrants further evaluation.
