? Aortic dissection.

Patient Data

Age: 25 years
Gender: Male

A well-defined soft tissue density lesion projected over the right side of the mediastinum. Right hilar vessels are clearly seen through the lesion indicating that the lesion is not in the middle mediastinum/hilum – hilar overlay sign.

CT study confirms the lesion at the right paravertebral location. It is a well-defined, well-margined benign-looking, low-density lesion close to dorsal vertebrae.

Case Discussion

This case is an example of the hilum overlay sign, a useful sign for the localization of thoracic lesions on frontal chest radiographs. The lesion turned out to be a benign schwannoma.
