Presentation

Pain and tenderness of the left midfoot for six months.

Patient Data

Age: 35 years
Gender: Male
x-ray
  • focal periosteal reaction at the second metatarsal mid-diaphysis more prominently on the medial aspect

  • subtle periosteal new bone on the medial aspect of the 4th metatarsal mid-diaphysis

  • no other abnormality

mri
  • fracture of the 2nd metatarsal mid-diaphysis associated with bone marrow edema displaying a low on T1 and bright on T2 fat suppression sequences in the metatarsal as well as surrounding soft tissue edematous changes

  • bone marrow edema along with the 4th metatarsal

Case Discussion

March fracture is a subtype of stress fracture that occurs secondary to repetitive minor trauma to a normal bone, classically the 2nd, and 4th metatarsals of the foot.
