Necrotizing retroperitoneal fasciitis

Patient Data

Age: 40 years
Gender: Male

Extensive bilteral ischioanal gas locules and fat stranding extend into the skin through an oozing sinus on the right side, tracking intra-abdominally as infected necrotizing gas-containing collections. These encircle the pelvic organs, extending at the mid-anterior abdomen just inferior to the umbilicus, and through the left paracolic gutter, inseparable from the descending colon and left ureter. The extension continues upward to the left subphrenic region, accompanied by surrounding phlegmon.

Non-enhancing fluid is observed along the left lateral abdominal wall.

Case Discussion

This is a case of maltreated perianal abscess, predisposing to the development of retroperitoneal necrotizing fasciitis. Urgent laparotomy and peritoneal lavage were performed; however, the patient passed away soon after the surgery.
