Pelvic fractures with intraperitoneal bladder rupture

Patient Data

Age: 20 years
Gender: Male

Bilateral superior and inferior pubic rami fractures, left posterior iliac wing fracture. Left sacral fracture.

CT cystogram confirms fractures seen on plain radiograph (bilateral superior and inferior pubic rami, left posterior iliac wing and left sacrum) and shows intraperitoneal bladder rupture with small defect in the dome of the bladder best seen in the coronal plane.

Post cystogram plain radiograph shows intraperitoneal contrast from bladder rupture.

Case Discussion

Typical findings of intraperitoneal bladder rupture with contrast freely flowing around bowel loops. The most common site of rupture (bladder dome) is well seen. Intraperitoneal rupture is surgically repaired.

Trauma patients often have multiple injuries: this patient also ruptured his left diaphragm (not shown).
