MRI demonstrates avulsion of the anterior-inferior labrum with the scapular periosteum remaining intact reflecting a Perthes lesion. Bony impression on the posterosuperior side of the humeral head (Hill-Sachs defect) with underlying mild bone marrow edema signal. No joint effusion.
Case Discussion
MRI findings are consistent with Perthes lesion and Hill-Saches defect. A Perthes lesion is one of the types of anterior glenolabral injuries, with a medially stripped but intact periosteum.
