Presentation

Under drug influence, tried to commit suicide a week ago by inserting a glass into his rectum.

Patient Data

Age: 35 years
Gender: Male
x-ray

Inverted drinking glass projected onto rectum. No free intra-abdominal air appreciated.
Feces distributed along the colon, no dilatation.

The glass was removed in the operating room under general anesthesia, after being divided into several pieces.

Post-surgical follow-up

Non-contrast CT before and after instillation of contrast material per rectum:
Gas bubble to the left of the rectum at 3 o'clock, which fills up with contrast material.
Thickened rectal wall.

Case Discussion

Young man with alcohol and drug addiction tried committing suicide a week earlier by inserting a glass into his rectum while under the influence of fentanyl.
In the operating room, under general anesthesia, the rectum was dilated but the glass could not be dislodged in one piece so it was carefully divided into several pieces and removed. An absorbable hemostatic gelatin sponge was left in place.
A CT was done several days later to verify the integrity of the rectum and showed indirect evidence of a minor laceration.
