Proximal pole nonunion fracture of scaphoid bone associated with secondary subchondral cysts at the proximal and distal poles are seen.

Fractured scaphoid with the proximal pole displaying low T1 signal near the fractured line. However, the presence of some more proximal areas that exhibits fatty marrow signal in T1WI suggests a still viable proximal pole.

Extensor carpi ulnaris (ECU) tendon contains small interstitial split.

Case Discussion

In the absence of post-contrast images, the presence of normal bone marrow signal in the proximal scaphoid pole on T1-weighted MRI without evidence of collapse or significant sclerosis is suggestive of scaphoid viability.
