Presentation

Sciatica.

Patient Data

Age: 55 years
Gender: Female
mri

A dome-shaped intraspinal extradural lesion at the left posterolateral aspect of L3 vertebral body with low signal on T1 and T2. It is compressing the left anterolateral aspect of thecal sac at the same level and occupies the lateral recess and extends through L3 exit foramen compressing left L3 exit nerve root. Features are suggestive of spinal meningioma.

L1/2 diffuse posterior disc bulge indenting ventral thecal aspect with no foraminal compromise.

L4/5 and L5/S1 diffuse posterior disc bulges indenting ventral thecal aspect and partially encroaching upon exit foramina bilaterally.

A small Tarlov cyst opposite S3 vertebral body is noted.

Case Discussion

Features are characteristic of spinal meningioma. Lumbosacral spine is the least common site for spinal meningiomas accounting for 5% of cases.
