Presentation

Sudden onset, shortness of breath in the absence of trauma.

Patient Data

Age: 35 years
Gender: Male
x-ray

Large right pneumothorax with collapse of the right lung. Left lung is clear. Tracheal deviation can be seen on the expiratory film.

x-ray

Chest drain inserted with good re-expansion of the right lung.

Case Discussion

This patient, a tall, skinny male with a background of Marfan syndrome and an active smoker, has multiple risk factors for spontaneous pneumothorax.

The expiratory film can be useful in detecting small pneumothorax. In this case of a large pneumothorax, it can reveal tracheal deviation not apparent on the inspiratory film.
