Presentation

Road traffic crash. The patient is admitted to the intensive care unit for an altered state of consciousness.

Patient Data

Age: 10 years
Gender: Female
ct

there is a transversal intralabyrinthine temporal bone fracture involving:

  • the jugular foramen

  • facial nerve canal

  • the otic capsule (cochlea, vestibule, and posterior semi-circular canal)

  • the temporomandibular joint

There is pneumocephalus and pneumolabyrinth. There is also air in the right jugular foramen suggestive of a jugular thrombosis.

Right occipital bone fracture.

Right parietal and occipital subcutaneous hematoma.

Bilateral frontal cerebral hemorrhagic contusion.

Acute bilateral frontal subdural hematoma.

Fluid opacification within the right mastoid cells.

Case Discussion

Right transverse intralabyrinthine temporal bone fracture with a probable jugular vein thrombosis (although mandatory to confirm the thrombosis, the contrast injection was not possible at that time).
